Responsible for coordinating psychosocial assessments and discharge planning for inpatient psychiatric patients at Northwest Hospital. Provides leadership and support to address individual patient needs, utilizing community and healthcare resources to ensure continuity of care post-discharge. Ensures timely and appropriate interventions to optimize resource utilization and patient outcomes.
Summary
JOB SUMMARY: Acts a coordinator of psychosocial assessment and discharge planning for the patients/clients receiving inpatient psychiatric services at Hospital, providing leadership and psychosocial support necessary to address the individual's needs. Acting as resource to patient/client, utilizes knowledge of community and health care resources to assist in addressing identified needs at any point of the continuum, re-admission to post-discharge. Using timeliness and appropriateness of intervention, facilitates quality resource utilization i.e. length of stay and level of care. REQUIREMENTS: Seasoned professional knowledge; equivalent to a Master's degree; knowledge in more than one discipline. Graduate of a Master's degree program in social work, psychology, or other appropriate degree. Less than one year of experience. Maryland Social Work licensure LMSW or LGPC.Additional Information
