We’re looking for a compassionate and skilled Physical Therapist Assistant (PTA) to deliver hands-on therapy services in the comfort of patients’ homes. Working under the direction of a Physical Therapist and following the physician-ordered Plan of Care, you’ll help patients regain strength, mobility, and independence while supporting their overall well-being. As a key member of our interdisciplinary team, you’ll work autonomously in the field, communicate important updates to the care team, and ensure therapy is both safe and effective in the home environment. As a member of the multi-disciplinary team, the therapist assumes responsibility for communication of client related issues for appropriate team members or other facility/agency staff and participates in the collection and evaluation of data necessary to assess the appropriateness of care at home. The PTA works with the clients and family in the home setting without direct supervision. Position Specific Responsibilities include delivering quality therapy services by implementing treatment plans established by the Physical Therapist, providing timely documentation and regular communication with clients and supervising PT, demonstrating clinical competency across all age groups, maintaining accurate documentation for clinical care, billing, and reimbursement, recommending appropriate medical and rehabilitation follow-up, adapting techniques and equipment for patient safety, independently managing caseload under PT supervision, organizing weekly schedules, demonstrating knowledge of interdisciplinary services and making referrals, participating in training and community education, meeting productivity standards, providing orientation to new team members, and providing consultation and training in settings beyond private homes. Education required is an Associate degree in Physical Therapy from an accredited institution, with preferred experience of 1 year in home care or rehabilitation. Required licensure is a current, unencumbered Physical Therapy Assistant license. Physical demands include standing, walking, manual dexterity, lifting up to 50 pounds, and exposure to typical healthcare conditions.
